Here are the forms to register for the 2011-2012 school year. Please note, registration forms and tuition deposit must be in on or before August 8, 2011.

The Excel worksheet will calculate the amounts automatically once completed and you can print them out and either mail to CCS, POB 447, Westfield, MA 01085 or drop off the form and deposit to the school.

My office hours are Mondays and Wednesdays from 9:00 a.m. to noon. If you want to come by the school, please either call me or email me with a date and time so that I can make myself available to assist you. I will also make the forms available at the school. Please contact me with any questions.

I would like to remind you to order your curriculum and to make sure that the ISBN# (usually located on the back of book with bar code) matches the number on the curriculum list. Let me know if you have any questions there.
